Lifewheel Rehabilitation LLC was founded in the fall of 2016 by practicing Physical Therapists who believe in patient centered care driven by the individual patient goals. Treatment plan is customized to meet the patient’s functional needs and to address their rehabilitation goals. We realize quite several individuals need Physical and Occupational therapy services; however, transportation could be a problem as well as other logistics which serves as a hindrance to them getting the services they need, to improve their functional potentials. Since we are caring and compassionate, we decided we will deliver quality rehabilitative care at the comfort of our patient homes.

We accept all major insurances- Medicare, Medicaid and most Private insurances. Self-pay option is also available.
